Case Management Services:

Prevention Case Management
Working on an individual basis, the Prevention Case Manager assists individuals who are at high risk for HIV transmission, using a variety of community and educational services, and works with the client to encourage an individual behavioral shift and develop a care plan to meet the client’s needs and goals.

HIV Case Management
Utilizing a wide variety of culturally appropriate services including health education, traditional healing, mental health and social services, the case management team assesses the individual needs of the client, and creates an individualized treatment plan with the clients. Issues addressed in this plan include treatment advocacy, access to medical care and services, substance abuse treatment, and housing.

Peer Advocacy
Provides individual practical and emotional support services and works closely with the Case Manager and Treatment Advocate to engage and maintain high risk, hard to reach clients in HIV care services.

Traditional Healing
NAAP created the traditional healing program to preserve Native tradition within an urban environment and to compliment the western care our clients receive. Through our traditional healing program clients are brought out to meet traditional healers, be apart of sweat lodge ceremonies, and attend local events such as pow wows. This connection to culture not only increases their spirits, but also encourages sobriety and improves medication adherence while educating and empowering our clients.

Mental Health
NAAP believes in serving not only the physical needs of our clients, but also in promoting mental and spiritual health.

Volunteer Program
Are you concerned about Native American health care and community access to resources in the SF Bay Area? Do you want to build your non-profit administrative skills, direct health, social media support, graphic design or outreach skills? We need your dedication and support to continue to build stabilization for the Native American community, specially our community members struggling with HIV/AIDS.
